Homecare Re-ablement
CSED's Homecare Re-ablement work seeks to improve choice and quality of life for adults who need care.
Approach
Evidence shows that timely bursts of social care re-ablement, focusing on skills for daily living, can enable people to live more independently and reduce their need for ongoing homecare support. Homecare re-ablement complements the work of intermediate care services to help:
- maximise independence
- minimise whole life cost of care.
Social care and health services will always need to make best use of limited resources. Demographic projections also show an increasing demand on all modes of care. The approach needs to ensure that the lowest appropriate level of intervention is provided set within an active and ongoing assessment process to balance risk against 'quality of life' for adults who need care. Other initiatives such as telecare services will also play a significant part in supporting people in their 'home', whether it be an individual residence, sheltered or extra care housing.
CSED's homecare re-ablement work focuses on four main projects
- Review of continued roll-out and support for re-ablement
- Prospective longitudinal study
- Homecare re-ablement for those on maintenance packages
- Post-initial homecare re-ablement phase
